Source and Methodology: Restaurants cited for the most total “critical” Health Code violations since 2005 according to the Iowa Department of Inspections and Appeals Web site.

Note: Some restaurants were inspected only once, and others as many as eight times. As a result, restaurants that were cited for as many as seven “critical” violations in a single inspection did not make this list. Yet some that did make the list were never cited for more than five code violations at a time. Frequent inspections added to their total and tilted the playing field. Restaurants are listed by the most total citations for “critical” Health Code violations. The numbers in parentheses are the highest number of violations in a single inspection at the restaurant.
